That is surprising. Since the issue is easily and 100% reproducible on TQ that suggests that the cause lies in the difference between TQ and your dev environment.
Let us know if we can be of any assistance to help you reproduce it or giving you the exact times of when we spawned wormholes using this bug so you can track it down in the logs, etc.
PS: Or just search for these types of log messages:

Hehe, **** happens, glad you figured it out.
We just had a possibly related bug: We collapsed a wormhole (by mass), but the signature did not go away. When you probe it the 100% hit is type "Unknown", Scan group "Cosmic Signature", Type "Unknown". When you warp there, you find nothing. --

Indeed, the reproducible case that I have seen is only with time expired wormholes. I will continue investigating and monitor the symptoms, if there are problems occurring with mass collapsed wormholes then that is a separate issue.
No, sorry if I was unclear, the reproducible bug that this thread is about indeed only applies to time expired wormholes.
We did however never experience the second bug I described before (Kacer Xenro might have more experience with it), i.e. a wormhole signature staying around (albeit named "Unknown" instead of "Unstable Wormhole") after it has collapsed AND disappeared from visible space. Since this is also a bug in the despawning mechanic I decided to post it here since it it might be related the first bug. In any case you are possibly just looking at the relevant code anyway. I have no reproduction steps at all, unfortunately, but Kacer Xenro's post above implies that it might be happening more often.
PS: Just like the first bug, the second bug also results in the signature disappearing over DT. --
